As a district, we have prioritized ensuring smooth, positive, and successful transitions for students as they progress between schools and grade levels. Significant work has been completed across all levels, focusing on maintaining continuity of learning, aligning instruction, and providing social emotional and academic support tailored to individual student needs.

At the Coventry Grammar School (CGS), strong progress has been made to maintain and utilize transition data, particularly for the Hale Early Education Center (HEEC) students entering Kindergarten and for Grade 2 students moving to Grade 3 at the G.H. Robertson School (GHR). Staff have implemented structured vertical teaming protocols to guide collaboration between sending and receiving teams, using transition data to create balanced classrooms that address academic, social emotional, and behavioral needs. Transition year teachers have also increased visits to partner buildings and classrooms to deepen understanding of instructional routines, expectations, and learning environments. Additionally, attention has been given to tiered supports and Universal Design for Learning strategies to ensure equitable access, predictability, and engagement for all learners.

At GHR, the transition into Grade 3 and out of Grade 5 is a historically challenging period, with achievement dips observed in the past. To mitigate these challenges, GHR has implemented protocols that include vertical meetings, 504 and PPT participation, and collaboration with sending and receiving teams to review individual student strengths and needs. Transition supports also include family orientations, scheduled for April 8 for incoming second grade families, and May 6 for rising sixth grade families and student visits to new buildings in June. Programming alignment, including Wit & Wisdom K-3, Acadience reading screeners, and i-Ready math interventions, has further strengthened continuity. Grade 5 math homework has also been aligned to support spiraling review in preparation for middle school expectations.

The district has enhanced transition support through the implementation of an aligned Multi-Tiered System of Supports (MTSS) structure and the integration of eduCLIMBER as a data warehouse. This allows seamless sharing of student information across schools, supporting targeted intervention and informed instructional decisions. Collaborative reviews between the Capt. Nathan Hale School (CNH), GHR, and Coventry High School (CHS) staff, supported by K-12 specialists in literacy, mathematics, and science, ensure a spiraling and rigorous curriculum aligned across all grade levels.

At CNH, the Student Support Team conducts comprehensive spring reviews of student profiles to tailor transition plans to individual needs. This year, rising eighth grade students participated in a course fair and small group tours to explore high school pathways. In collaboration with GHR, rising fifth grade students engaged in a new transition experience designed to build early engagement and social emotional readiness for middle school.

CHS continues to partner with CNH to support students transitioning to high school. As noted earlier, counseling and teaching staff provide course fairs, program reviews, individual and small group tours, and meetings with students and families. CHS counselors and case managers meet with CNH counterparts to identify students who may need additional support, ensuring continuity of academic, social emotional, and behavioral interventions.

Collectively, these initiatives demonstrate the district’s commitment to proactive, data informed, and collaborative practices that support student success at each stage of the K-12 progression, fostering continuity, preparedness, and confidence for students and families.
